Duplicate

To request a duplicate, please complete the duplicate form and have it signed by a veterinarian or passport consultant and submit it with the documents we have requested. Upon return of the completed form we will assess whether we can process your request. Should this not be the case, you will receive a message from us explaining why the application cannot be processed.

If the application form is complete and all documents have been submitted, you can receive the document in question within a few weeks. All costs to obtain a duplicate are the sole responsibility of the applicant.

When sending this application form, please attach clear color photos of the horse in question, namely:

• Van de gehele pony
• De voorzijde van het hoofd
• De benen
• Bij ontbreken van de foto’s kunnen wij de aanvraag niet in behandeling nemen.
• De foto’s worden niet aan u geretourneerd, zij blijven als “bewijsmateriaal” in ons bezit.

Voor een snellere afwerking verzoeken wij U een plukje manen in een haarzakje te retourneren met de aanvraag ( voor eventueel DNA onderzoek).

The NWPCS is entitled to require a DNA test before proceeding to issue a replacement registration certificate/horse passport to the applicant.
